General Forms

• Office of Registrar
  Refer to the Office of Registrar for a comprehensive list of forms for both undergraduate and graduate students. 

• Student Affairs
  Refer to the Office of Student Affairs' page for a list of forms pertaining to academic support, dean of students, and residence life.

• Course Withdrawal Form
  This form used to withdraw from a course (drop with a W) during the period designated in the Academic Calendar. It should not be used to drop a course during the add/drop period; that form is above. It is a new form that uses electronic signatures via DocuSign. Online help for DocuSign is available via LTS. 

• Petition to the Committee on the Standing of Students (SOS) 
  Students who need an exception to a current policy may submit a petition to the SOS Committee. For more information on the SOS committee visit their website at https://studentaffairs.lehigh.edu/.

• Degree Program Exception Approval Petition
  This form is used to petition for an exception to program requirements, and requires signatures from advisor, department chair and associate dean. 

• Undergraduate Petition to Reserve Courses for Graduate Degree 
  If you are taking graduate courses not needed to meet your undergraduate degree requirements AND you’re planning on continuing in Lehigh’s graduate program, you may complete this form to reserve courses.

• Undergraduate Student Approval of Transfer Credit 
  Current undergraduate students who plan on taking a course elsewhere should complete this form and submit with a course description to the academic department for review.

CSE Forms

• CSE Prerequisite Waiver Form
  For students who need to request a waiver for course prerequisites. Provide details about your relevant experience or coursework that justifies the waiver.

• Engineering 4+1 Declaration Form
  For current undergraduate students who wish to join the 4+1 degress program. Requires signatures from student's undergraduate advisor and the faculty graduate program director of tthe student's chosen master's program.

• Indpendent Study Form
  For students who are looking to propose an independent study project. Include your study objectives, a detailed plan, and the name of the faculty advisor supervising your work. Approval from the department is required before commencing the study

• Computer Science Minor Declaration Form
  This form is used for students who are looking to pursue a minor in Computer Science. The CS minor is not available to majors in Computer Engineering, Computer Science, nor Computer Science and Business. • Data Science Minor Declaration Form
